and experienced Home Health Physical Therapy Assistant (PTA) to
join our team in Portland, OR. The ideal candidate will be
responsible for providing high-quality rehabilitative services to
our patients in their homes. This is a full-time position and
flexible with your schedule!Competitive compensation ranging from
$79,000-$88,000/ year. (DOE)Territory: East PortlandRequirements:-
Graduate of an accredited physical therapy assistant program with
either an Associate's degree in Science or Applied Science approved
by the American Physical Therapy Association.- Current
certification or license as required by state law.- Possesses and
maintains CPR certification.- A licensed driver with an automobile
that is insured in accordance with state/or organization
requirements and is in good working order.Responsibilities:-
Provide physical therapy services to patients in accordance with
the established plan of care.- Assist physical therapists in
developing and implementing individualized treatment plans.-
Monitor patient progress and adjust treatment plans as necessary.-
Educate patients and their families about home exercise programs
and activities of daily living.- Maintain accurate and up-to-date
patient records.- Collaborate with other healthcare professionals
