Job Description:
The caseload is split roughly 50/50 between Salud Medical Center
in Woodburn, Oregon, and Lancaster Family Health in Salem, Oregon.
Mileage between work sites is reimbursed.
Assigned clients will be within the Community Health Services
Department with Maternity Case Management program and could include
issues with grief, anxiety, depression, trauma, and people with
emotional, behavioral, and social needs. In this role, you'll
provide mental health services as determined in the treatment or
care plan of the client. Support patients and families as they
navigate medical challenges, process complex information about
pregnancies and neonates, and access community supports. When
perinatal loss occurs, helps families to understand, express, and
cope with feelings of grief
- This position includes home visiting as part of the essential
functions.
- Bilingual English/Spanish is strongly preferred. Differential
pay with passing score on ALTA language scale. What We Offer
- MHW III salary $87,500; MHW IV salary $100,000.
- $5,000 sign-on bonus in first paycheck and$1,000bonus at 12
months of employment.

Essential Functions/Responsibilities/Duties
- Assesses initial client treatment needs and formulates an
appropriate diagnosis. Provides treatment to patients with more
complex or multiple diagnoses. Performs intake session with clients
and completes Integrated Biopsychosocial Assessment.
- Develops treatment plan incorporating standard practice
guidelines for each diagnosis. Provides therapy, supportive
therapeutic counseling, and education to assigned individuals.
- Applies therapeutic treatment models, such as behavior
modification, cognitive behavioral therapy, and brief therapy
during counseling sessions.
- Develops cross system plans or collaborates with other
community professionals in the treatment of individuals.
- Provides cognitive behavioral therapy to individuals, families,
groups, or communities as assigned. Integrates cognitive behavioral
interventions in multiple settings.
- Responds to crisis situations, assessing urgency of patient's
needs, provides care, and obtains appropriate services.
- Provides training and education on clinical topics to team
members, staff and the community, as needed.
- Performs clinical responsibilities in alignment with The Joint
Commission (TJC), Health Resources and Services Administration
(HRSA), and YVFWC requirements.Qualifications
- We will hire either level III or level IV, depending on the
qualifications of the person hired:
Mental Health Worker III Requirements
- Education:Master's Degree in Social Work, Psychology,
Counseling or related field.
- Preferred Experience: Two years' experience in the direct
treatment of persons with mental illness or emotional disturbance
that was gained under the supervision of a mental health
professional. One year's full-time experience in the treatment of
children under the supervision of a children's mental health
specialist.
- Licenses/Registration:One of the following licenses is
acceptable for this position (must apply for within one week of
hire if license is not current):
- Certified Social Worker Associate (CSWA)
- Licensed Master's Social Worker (LMSW)
- Basic Life Support (BLS) certification within 45 days of hire.
Mental Health Worker IV Requirements
- Education:Master's Degree in Social Work, Psychology,
Counseling or related field.
- Experience: Two years' experience postgraduate/master's
experience in the direct treatment of persons with mental illness
or emotional disturbance; such experience must have been gained
under the supervision of a mental health professional.
- Licenses/Certificates/Registration: One of the following
licenses is required for this position (must apply for within one
week of hire if the license is not current): Oregon State Board of
Licensed Social Workers
- Licensed Clinical Social Worker (LCSW)
- Licensed Master Social Worker (LMSW)
- Licensed Professional Counselor (LPC)
- Basic Life Support (BLS) Certification obtained within 45 days
of hire.Additional Requirements: Bilingual (English/Spanish)
