BONUS!JOB SUMMARY::A Licensed Practical Nurse (LPN) working in our
Skilled Nursing facility is responsible for the daily nursing care
needs of our residents. This position is a valuable clinical team
member contributing to the smooth, organized and efficient
operations of the nursing department.RESPONSIBILITIES:* Provide
primary nursing care to residents with varying backgrounds and
medical histories* Assists with new admissions* A Licensed
Practical Nurse (LPN) will monitor and communicate changes of
condition to provider* Provide medical intervention as needed for
emergent needs* Communicate needs of resident to outside providers
including but not limited to: Primary Care Provider, Home Health
Agency, Pharmacy, etc.* As a Licensed Practical Nurse (LPN) you
would communicate resident condition to family, and or responsible
party members* Implement and follow Primary Care Providers orders*
Other Duties as AssignedREQUIREMENTS:Licensed Practical Nurse (LPN)
